- Implement and monitor patient care plans, and monitor, record, and communicate patient conditions as appropriate.
- Serve as the primary coordinator across disciplines to support well-coordinated patient care.
- Record and carry out physician and nursing orders.
- Assess and coordinate patients’ discharge-planning needs with members of the healthcare team.
