Schedule: Monday to Friday, 8:00–16:30 (40 hours per week). Some holidays and one weekend per month may be required.
Requirements: Two years of experience, including experience in a skilled nursing facility (SNF).
Responsibilities:
- Perform initial and ongoing assessments of patients’ conditions.
- Establish, revise, and evaluate appropriate plans of care for identified needs in collaboration with patients and their families.
- Provide therapy interventions using standard physical therapy techniques and skills appropriate to each patient’s condition.
- Oversee the activities of physical therapy assistants, students, and other support personnel.
