Schedule: Monday to Friday, 8:30 a.m.–5:00 p.m. (8-hour shifts; 40 hours per week). Some holiday work and one weekend per month are required.
Requirements: 2 years of experience, including experience in a skilled nursing facility (SNF).
Responsibilities:
- Perform initial and ongoing assessments of patients’ conditions.
- Establish, revise, and evaluate plans of care based on identified needs, involving patients and their families.
- Provide appropriate physical therapy interventions based on each patient’s condition.
- Oversee physical therapy assistants, students, and other support personnel.
