Schedule: Three 12-hour shifts per week, 7:00 a.m.–7:30 p.m., Monday to Friday (36 hours per week)

Responsibilities:
- Implement and monitor patient care plans; observe, record, and communicate patient conditions as appropriate.
- Coordinate care across disciplines to support well-coordinated patient care.
- Carry out physician and nursing orders.
- Assess and coordinate patients’ discharge planning needs with the healthcare team.
